Service Plan Activation Form

To get started please complete the Service Plan Activation Form below. Please have the IMEI number handy. The IMEI number can be found in several places including the cover of the welcome envelope, the getting started letter and on the bottom of The Guardian control unit. You will need to complete all fields with the “*” in order to submit the form.

Once we receive your information, we will setup your dashboard and email the annual service plan invoice to you.

After you make payment, we will contact you to schedule an overview of the dashboard app.

In the meantime, you can download the mobile app from the app store on your phone. Type “TMG DASH” to find the app.
